Cyclone in the “Malvinas Islands” and the Spiral Galaxies

Abstract:

A subtropical cyclone is a weather system that has some characteristics of a tropical cyclone and some characteristics of an extratropical cyclone. They can form between the equator and the 50th parallel. In mathematics, a spiral is a curve which emanates from a point, moving farther away as it revolves around the point. The characteristic shape of hurricanes, cyclones, typhoons is a spiral. There are several types of turns, and determining the characteristic equation of which spiral the Extratropical Cyclone (EC) fits into is the goal of the work. The study demonstrates a double spiral for the extratropical cyclone, similarly Lindblad (1964) demonstrates a double spiral, to demonstrate the structure of spiral galaxies. Despite the data obtained in the EC that passed through the southern tip of South America west and east of the Falklands Islands, everything indicates that the short occurrence ECs indicate the double spiral structure, but with the structure of a C...

The Impact of Chest Computed Tomography in A Covid-19 Reference Hospital - First Wave - Distrito Federal - Brazil

Abstract:

Objectives

To evaluate the diagnostic accuracy of chest CT for the diagnosis of COVID-19 associated with the clinical presentation and in relation to the PCR-RT. Sensitivity, specificity, positive predictive value and negative predictive value, gender, age group and degree of lung involvement will be evaluated.

Methods

We evaluated 1545 patients with chest CT, delineating the age range and degree of lung involvement, and 306 patients with chest CT and PCR-RT.

Results

Of the 1545 examinations, 53% were men and 47% were women, there was greater involvement in the 50-59 age group. In the pulmonary study, 55.05% were COVID-19. In the degree of lung involvement 37.70% were mild, 35.76% were moderate, and 26.54% were sev...

Outcome of Percutaneous Coronary Intervention Among Very Elderly Patients with Ischemic Heart Disease

Abstract:

Background

Cardiovascular disease, and ischemic heart disease (IHD), is a major cause of morbidity and mortality in the very elderly patients worldwide. In the developing countries like Bangladesh it has been increasing with time. Due to life style, food habits and urbanisation. These patients represent a rapidly growing cohort presenting for percutaneous coronary intervention (PCI), now constituting more than one in five patients treated with PCI in real-world practice.

Materials and Methods

From July 2020 we included 152 patients with IHD purposively in Cardiology department of Bangabandhu Sheikh Mujib Medical University Dhaka, Bangladesh undergone PCI who were divided into 2 groups according to age: e” 75 years (n = 51) and <75 years (n = 101). Baseline clinical characteristics, indications for coronary int...

Cross-Reactivity between COX-2 Inhibitors in Patients with Cross-Reactive Hypersensitivity to NSAIDs

Abstract:

The safety of cyclooxygenase (COX)-2 inhibitors has been tested in patients who had cross-reactive hypersensitivity reactions (HSRs) to nonsteroidal anti-inflammatory drugs (NSAIDs). However, these studies have been mainly done before the current classification of NSAID hypersensitivity and cross-reaction between COX-2 inhibitors has been rarely reported. We aimed to assess tolerability of COX-2 inhibitors and to evaluate the cross-reactivity between them in cross-reactive phenotype of NSAID hypersensitivity. The diagnosis was based on clinical features, reliable history of HSRs to at least two chemically different NSAIDs, and/or positive provocation tests with implicated NSAIDs in 151 patients. Single-blind, oral challenges with 1/4 and 3/4 divided doses of placebo, nimesulide, meloxicam, and celecoxib, as COX-2 inhibitors, were performed. The most common cross-reactive phenotype was NSAID-induced urticaria/angioedema (56.3%). Positive reactions to...

A Clinical, Electrocardiographic and Echocardiographic Comparison of Patients with Single Vs Multivessel Disease Presenting with Acute Coronary Syndromes

Abstract:

Background

Prevalence of coronary artery disease is between 7-13 percent in urban and 2-7 % in rural India1. The alarm in rise in the prevalence of coronary risk factors like diabetes, hypertension, dyslipidemia, smoking, central obesity and physical inactivity2. The correlation between these risk factors and the severity of coronary atherosclerosis, assessed by angiography which may be either single or multivessel is less consistent with studies reporting conflicting results 3,4. Therefore our study aims to understand the proper correlation between risk factors and severity of coronary artery disease in an Indian population

A new improved method for preparation of 1,2-diacetyl D-ribofuranose derivative was developed via acetolysis of tri-O-pivaloylated D-ribofuranoside and plausible mechanism of the reaction was proposed. Synthesis of 3′,5′-di-O-pivaloyl-2,6-dichloropurine β-D-riboside along with isomeric 2′,5′-di-O-pivaloyl nucleoside was carried out by stereoselective glycosylation reaction of 2,6-dichloropurine with peracylated D-ribofuranose followed by regioselective 2´-O-deacetylation of protected β-ribonucleoside with different bases. Mild C2′-β-fluorination of the purine 3′,5′-di-O-pivaloyl ribonucleoside with an excess of diethylaminosulfur trifluoride afforded protected 2,6-dichloropurine 2′-fluoro β-D-arabinoside as the key intermediate.
